Intake & Outreach Coordinator- Sacramento

PURPOSE of ROLE: This position is responsible for answering calls, retrieving information and logging into HMIS database to screen individuals who are homeless for access to the shelter. This position provides information, referrals as appropriate, and inputs information into database.  This position also allows time for standard “street outreach” and acts as point of contact toe homeless community we serve.  This will include engaging homeless populations where they congregate to educate, build rapport and encourage individuals to get help at the shelter. 

RESPONSIBILITIES:

SCREENING, ASSESSMENT & TRACKING

Screens phone calls, offers information and referrals to other SHELTER, Inc. programs and other community agencies.

Refers clients to all appropriate public benefit programs, including providing information about how to contact applicable Homeless Coordinated Entry System.

Collects and documents eligibility information.

Regularly reports participant situations to case managers and supervisor.   

ADMINISTRATIVE

Answers telephone and directs calls, greets visitors at front desk as needed.

Performs other duties as assigned.

Attends SHELTER, Inc. internal meetings and outside trainings as assigned.

Help with both walk-in and call-in client needs.

Provides information regarding services provided by SHELTER, Inc.

Retrieves messages from voice mail and documents calls. 

OUTREACH

Plan, develop and implement outreach initiatives.

Follow up as required and needed.

Enter data into HMIS system.   

Answers questions, provide information and referrals as needed.

Seek out, speak to, build relationships and educate homeless individuals.

Other duties as assigned.

Basic competency in data entry. 

MINIMUM QUALIFICATIONS:

Experience working with low income and/or homeless populations.

Computer skills including experience with Microsoft Word, Outlook and Excel.

Type at a minimum of 35 words per minute required and validated with certificate.

If required to drive personal vehicle on behalf of agency business must have a DMV record that permits driver to be insured under SHELTER, Inc.’s automobile coverage.

Must successfully pass a criminal background check.

PREFERRED QUALIFICATIONS:

Associates degree in social services or related area, or certification from a two year program in related area.

Bilingual English/Spanish.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
